Student Name: Emily Stong Dissertation Title: Evaluating Expenditure per Patient on Treatment and Viral Suppression Among Local Versus International PEPFAR Partners Across Four Operating Units, FY2018–2023 Dissertation Chair: Dr. Kathleen Baggett Dissertation Abstract: Introduction: PEPFAR has increasingly shifted funding and implementation responsibility to local organizations as part of its broader focus on sustainability and country ownership. However, there is still limited evidence comparing local and international implementing partners across multiple countries, particularly in terms of expenditure and program outcomes. Methods: This study used PEPFAR Monitoring, Evaluation, and Reporting (MER) and Expenditure Reporting (ER) data from Kenya, Zambia, Eswatini, and Uganda from FY2018 through FY2023. The analysis compared local and international implementing partners on two outcomes: expenditure per patient on treatment (TX_CURR) and viral suppression (TX_PVLS). Descriptive analyses and regression models were used to compare partner types while accounting for operating unit and fiscal year. Results: After adjustment for operating unit and fiscal year, partner type was not significantly associated with either expenditure per patient or viral suppression. Local partners had an estimated 24.2% higher expenditure per patient than international partners, but this difference was not statistically significant. Expenditure per patient differed across operating units and generally declined over time. Viral suppression also differed across operating units and improved over the study period. Conclusion: This study did not detect statistically significant differences between local and international implementing partners in either expenditure per patient or viral suppression. Differences across operating units and fiscal years were more apparent than differences by partner type. These findings provide a baseline for future comparisons of partner performance as PEPFAR localization continues.
